Referral Process and Forms
Begin the referral process for one of your patients.
GPs may refer directly to specialists at IntraCare for selected interventional radiology procedures with a letter of referral.
Otherwise, you can refer directly to Specialists’ private rooms through Healthpoint or Healthlink.
For open-access referrals, please complete the online form below.
Referral Form
This referral form needs to be completed for referrals so IntraCare can process the booking:
- All fields within the form are mandatory.
- Please ensure that the patient is aware of the referral to IntraCare.
- The request is required a minimum of 7 business days prior to the procedure.
